Only fill this form out ONCE. If you have already filled it out - STOP. You are in our system and we can find you without the card.

TO SIGN THE CONSENT FORM - Be sure you TYPE your name EXACTLY as it appears on the pop up screen. Your phone puts a space after your last name sometimes - do not let his happen as your signature will not take.

After you complete the form, you will receive a membership card to the pantry. BE SURE TO SAVE THIS CARD and show it every time you come to shop for faster service! You do not NEED to register again once you have this card.

IF YOU DO NOT GET THE MEMBERSHIP CARD - YOU ARE STILL IN OUR SYSTEM. Come to the pantry during our open hours and we will help. DO NOT KEEP FILLING THIS OUT. PLEASE.

You can shop every 2 weeks.
